How to submit your request
Prepare your documents
You’ll need to provide a digital copy of one of the following documents:
- Name Change Certificate
- Birth Certificate
- Driver's License
- Passport
Note: Please ensure that the documents are clear and legible.
Submit your request
Online form: Fill out the form below. You’ll need to include your account number to ensure that your request is processed efficiently. Once you’ve filled out the form and attached all required documents, submit your request.
Note: you can find your account number in the top left corner of your PDF bill.
